The NP is accountable for the management and evaluation of a specific panel of homeless patients under his/her care. The NP provides comprehensive preventative and wellness care, as well as the spectrum of acute, episodic, chronic and end-of-life care to homeless Veterans.
The NP is a member of the core Homeless Patient Aligned Care Team (H-PACT), patient, provider, RN Care Coordinator, Licensed Vocational Nurse, and Medical Administrative Support Clerk. The NP uses the nursing process and evidence-based practice to provide diagnosis and treatment of patients and to work collaboratively with the expanded team to include family/caregiver, internal VA specialists and services, as well as with community-based health care services involved in providing care to the homeless Veteran patient. The NP will develop a patient-driven holistic care plan for life for his/her patients. The NP provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The NP is prepared in advanced practice across the health continuum. This practice includes independent and interdependent decision-making and direct accountability for clinical judgment. The NP role includes participating in and use of research, development and implementation of health policy, leadership, education, collaborative management and consultation. The NP provides care in collaboration with various community-based organizations throughout West Contra Costa County and Northern Alameda County.
